To answer all of your questions...

I am an engineer and have worked as a metrologist (which is different
from a meteorologist), so should know something about measurements.

Magnetic north would be determined by a special hi$$$ 'compass' that
the survey team would carry around with them.

As far as northern Michigan is concerned, I would think that what ever
the local magnetic field at the airport is, that would be how the runway
is alligned.

Not only is the pole changing, but the magnetic field itself can change
independently of the pole position.

Once a survey is done of an area, then the info can be passed on to the
chart makers.  I am sure that the isogonics on the charts are more of an
average, since I find it hard to beleive that they could really be that
smooth.
