John,

Been there, done that.  Fortunately my CFI signed me off, I
passed my flight test, and we came into some extra money not
too long after that so I could keep flying.

IMO, you should discuss your situation first with your CFI.
Tell him where you're at and ask him to tell you very specifically 
what changes he wants to see before he'll sign you off for your flight
test.  He should be able to tell you specifically, and you can focus
there and hopefully put extra effort there and succeed.

If he's flown with you 20 hrs, he should have plenty of idea
what kind of pilot you are.

If your CFI can't tell you specifically or is ready to leave without 
signing you off, IMO talk with the Chief Flight Instructor and 
Manager or Owner of your FBO.  Let them know that you consider
instructor turnover has been a big factor in your predicament, 
that you have all the requirements finished, and that you would
like them (after you being such a loyal customer for 80 hrs and
4 CFI changes) to help you out.  Ask for a reduced rate (block
rental rate with no block); ask the chief CFI or the manager to
fly with you once and see if they feel they would be able to
sign you off, or place you with a CFI who is comfortable signing
you off, with the minimum review.  

If this doesn't work, don't give up.  Find out if there's a local
or state CFI association, call the director, and tell your story
there.  He may be able to put you in touch with someone who will
help you out for the love of flying. 

Don't give up.  You've come so far, you've done so well, fight
for it.
