I'm not sure what "Phase of Flight" formation flight would be
(maneuvering?) so I might have selected them out by rejecting
"cruise" and "maneuvering" phases of flight. 

There was one accident involving two aircraft (not in formation)
which took off from the same airport and departed in the same
general direction.  One overtook the other.  The aircraft were
in radio communication, and the overtaking aircraft informed
the other that he would pass on the left and received an
acknowledgement. (This is NOT standard, but was not a cause of
the accident).

They collided when both were distracted by a twin engine aircraft
flying towards them near their altitude.  Moral, don't cut
corners when you're passing someone, give them enough berth to
tolerate some distraction.
